Home Case Management  Services

Our Care Managers provides our members with the tools and immediately accessible information they need to obtain the best results  for our members. Our care managers:

  • assesses, plans, implements, coordinates , monitors, and evaluates the options and services required to meet the members health and human service needs

  • assures that members reach their optimum level of wellness, self-management, and functional capability

  • facilitates the achievement of member wellness and self-sufficiency through encouragement,assessment, planning,communication, education on resource management

  • links members with appropriate providers and resources throughout the continuum of health and human services and care settings,while ensuring that the care provided is safe ,effective,member-centered,timely,efficient, and equitable

  • enhances these services by maintaining the member’s privacy confidentiality, health,and safety through advocacy and adherence to ethical,legal,accredited,certified,and regulatory standards or guidelines

Case Management

We provide a full comprehensive assessment, develop/ service plan in collaboration with the clients’s physician or relevant provider, family and health care team to achieve the wellness goals.

  • Identify and facilitate clients access to community based services and entitlements.
  • Coordinate all services to assure efficiency, appropriateness, timeliness and quality of care, consistent with the care/service plan.
  • Identify risk factors including medical, psycho-social, environmental and financial.
  • Monitor  the clients stats modify plan based on changes in stats and needs. Remain in contact with client and care team as need
  • Provide health education to enable client to improve his/her ability to manage his/her health.

Case Management Home Visits

 the case management services will help clients;

  •  Be empowered to maintain their well-being in the comfort and familiarity of their own homes.

  • Avoid unnecessary ER visits and hospitalization

  • Have the means to obtain medications, understand what medications to take and know how to take them

  • Understand self-care requirements and be able to implement them’

  • Have access to helpful resources and a support network as needed

  • Access primary care physician care and follow-ups as required

  • Understand the benefits of preventive health care and keeping regular appointments with their  primary care physician
